Records and Transcripts

The Office of Rights, Compliance and Accountability maintains a disciplinary record for every student found responsible for a violation of the University Student Code of Conduct. Disciplinary files are considered educational records and cannot be provided to outside parties without the student's permission. However, some employers and graduate and professional schools require that applicants authorize the release of their disciplinary records in order to be considered for employment or admission. A Release Authorization Form is required to release a student's disciplinary file.

The files maintained by the Office of Rights, Compliance and Accountability are separate from transcripts, which are maintained by the registrar's office. If a student is suspended or expelled for a disciplinary infraction, that sanction will be noted on his or her transcript. A suspension is noted only during the suspension period; once the suspension has been served, the notation is removed from the transcript. Expulsions are permanently noted, except in certain cases where clemency has been granted.

Requesting Disciplinary Records

Student:

A student may request a copy of his or her disciplinary record or request that the record be provided to a third party by completing a Release Authorization Form. The completed form should be emailed to the Office of Rights, Compliance and Accountability at uaf-studentrights@alaska.edu, faxed to (907) 474-7900 or mailed to:

Agency:

An outside agency may request a copy of a students disciplinary record by sending their Authorization for Release form. Please submit the Authorization form to the Office of Rights, Compliance and Accountability at uaf-studentrights@alaska.edu, faxed to (907) 474-7900, or mail to:

Processing Time

Processing time to review a student conduct record is up to 45 calendar days.

Processing time for agencies to receive a copy of all disciplinary records is two business days from the day we receive the request. There are NO exceptions that will be made to this policy.
